Having Trouble?

Please make sure that you save the survey form to your hard drive before beginning.

Common problems:

I try to save the survey form and I’m being asked for a password.

If you are being asked for a password, you have “left-clicked” on the link for the survey form. To avoid this error:

  • Right-click on the link (usually this is done by using your middle finger to click the button on the right-hand side of your mouse).
  • Choose “Save Target As.”
  • Save the survey form to your desktop or some other place where you can easily locate it.
  • Complete the survey form on your own time, saving your changes as you go.
  • Submit your survey using the instructions in the survey form.

I opened the survey form, completed some or all of the survey, and now I can’t save my changes.

This is not an online survey, so you will not be able to save your changes if you open the survey form directly from the Web site (by left-clicking and choosing “open”). Left-clicking and choosing open causes your Web browser to open the survey form, and the survey form does not work correctly. One side-effect of this is that you will not be able to save it to your hard drive or send it as an e-mail attachment.

If you have completed portions of the survey form:

  • Print out the form.
  • Download the survey form following the instructions above.
  • Re-enter your information into the survey form.
  • Submit your survey using the instructions in the survey form.

If you completed all of the survey and cannot save it, please print it out and submit your survey by mail using the instructions in the survey form.
